Data that has been merged from disparate sources – whether internal, external or the result of M&A – will only be actionable if accurate and complete identity has been established of the people you serve. And in the absence of a national patient identifier, health systems have been left to their own devices to ensure those records are properly matched, avoiding the all too common mistakes in which the records of different patients are merged, or a new record is created instead of an existing one being found. But identity resolution errors abound, impacting patient safety and quality and cost of care. In this important webinar, we’ll hear from leaders who have made progress towards their patient identity and matching goals so the fruits of their interoperability work can be realized.
