Most technology-based cyber-defenses become obsolete the moment a user gets fooled and gives up their credentials, said FBI Supervisory Special Agent David Fine, during his presentation at the recently-held HIMSS Healthcare Cybersecurity Forum, which makes it absolutely incumbent on security executives to shore up their anti-phishing training based on the psychological susceptibilities bad actors are leveraging.
He delved into the dark world of social engineering, explaining how cyber adversaries manipulate human psychology to exploit vulnerabilities within organizations. Fine, with years of experience in cybersecurity investigations, emphasized that the biggest weakness in any security infrastructure isn’t just technology but the people using it.
Fine outlined social engineering as the act of manipulating people into performing actions they wouldn’t normally take, or revealing confidential information. Contrary to popular belief, these cyber-attacks do not rely heavily on technological loopholes or unpatched software; rather, they prey on human psychology. When it works and good people are tricked, “All the money and all the processes in the world go by the wayside,” he explained.
Phishing, Fine stated, remains the most prevalent form of cyber-attack. Traditionally, phishing attempts were straightforward email scams, often easy to spot. Today, however, phishing has grown more sophisticated, incorporating various communication technologies and appearing as legitimate messages. Fine noted that the FBI has documented billions of dollars lost to business email compromise, where attackers impersonate trusted parties to manipulate employees into transferring funds. “If a communication technology exists, it is being used for phishing,” he said, noting that attackers now utilize methods like QR codes embedded in legitimate-looking PDFs to avoid detection.
Fine stressed that attackers exploit unconscious biases and cognitive shortcuts that shape our decision-making. The human brain, he explained, relies largely on “System 1” thinking—rapid, instinctual responses—to process overwhelming information quickly. Attackers craft their messages to keep recipients in this automatic thinking mode, making it less likely that they will question the message’s legitimacy.
Through real-life examples, Fine illustrated how attackers leverage trust, authority, and familiarity to influence their targets. When people see an email that looks like it’s from a trusted source, their initial reaction is not skepticism but compliance. This automatic trust, he said, can be a dangerous vulnerability, especially if the email appears to come from a familiar source, like a boss or a known vendor.
Drawing from Dr. Robert Cialdini’s work on persuasion, Fine highlighted six principles that cybercriminals frequently exploit:
- Reciprocity – People feel compelled to return favors.
- Scarcity – A perceived shortage increases the desire to act.
- Authority – Messages appear more credible when they seem to come from a figure of authority.
- Consistency – People strive to be consistent with their past actions and commitments.
- Liking – People are more likely to comply if they like the person making the request.
- Social Proof – Individuals are influenced by observing others’ behavior.
These principles, used in combination, create powerful persuasion traps. A well-crafted phishing email might play on authority (appearing to be from an executive), scarcity (limited time to respond), and social proof (mentioning that “everyone else” is doing it), making it difficult for recipients to resist.
Fine warned about the next generation of cyber threats, which utilize AI to enhance deception. He mentioned a recent case where attackers used deepfake technology to impersonate an executive, ultimately tricking an employee into transferring $622,000. Additionally, attackers now use QR codes, cloaked in legitimate documents, to lure users into scanning and providing access to sensitive information. “These attacks hide the malicious behind the benign,” Fine noted, underscoring the sophistication of today’s cyber threats.
While the threat is ever-evolving, Fine outlined key strategies for defense:
- Promote a Culture of Vigilance – Organizations should foster an environment where employees feel safe reporting suspicious activities, even if they initially fell for them. This transparency can be crucial in responding swiftly to an incident.
- Encourage System 2 Thinking – Slowing down is often the best defense against phishing. By taking a moment to scrutinize emails and messages, employees are more likely to recognize red flags. Fine urged employees to cultivate “judicious skepticism” in their interactions.
- Implement Out-of-Band Verification – Establishing a separate verification method, such as calling the sender or using a secure internal platform, can help confirm whether a message is legitimate.
Fine emphasized that while technology plays an essential role in cybersecurity, human behavior is often the weakest link. Cyber threats, especially those employing social engineering, will continue to evolve, but organizations can protect themselves by promoting awareness, encouraging vigilance, and implementing simple but effective verification processes. In Fine’s words, “We need to focus on the people if we want to stand a chance of fixing the problem.”
